I love The Templeton on Granville. They sundaes aren't *crazy* good... but it's a heritage 50s diner withe a jukebox at every table. If you're going to do sundaes then you should really do the whole theme :) oh.. and Banana slits too!

Death by Chocolate on West
Death by Chocolate on West Broadway has fairly decent sundaes. As expected, the chocolate sauce is the best part.
